What Is Medicare Part D?

Medicare Part D is your prescription drug coverage. It helps pay for the medications your doctor prescribes—whether you take one pill or several. Unlike Parts A and B, Medicare Part D is offered by private insurance companies approved by Medicare. If you have Original Medicare, you’ll need to add a standalone Part D plan to get drug coverage. Each Part D plan has its own list of covered medications (called a formulary) and a network of pharmacies. Plans vary by ZIP code, so your choices depend on where you live.

What Do Medicare Drug Plans Cover?

Medicare Part D helps pay for prescription drugs, but coverage and costs can vary widely between plans. Each plan uses a formulary (a list of covered drugs), and those drugs are grouped into tiers that affect how much you pay:
  • Tier 1: Most generic drugs — lowest copays
  • Tier 2: Preferred brand-name drugs — moderate copays 
  • Tier 3–5: Non-preferred, specialty, or high-cost drugs — higher out-of-pocket costs

Part D plans may also include rules like: 

  • Prior authorization
  • Step therapy
  • Quantity limits

What Are the Costs of Medicare Part D?

Monthly Premiums

Most people pay a monthly premium, which can differ based on the plan you choose.

Annual Deductible

Some plans have a deductible before coverage kicks in (capped at $590 in 2025).

Copays or Coinsurance

After meeting your deductible, you’ll pay a portion of your drug costs, depending on the tier of the medication.

When Can I Enroll in a Medicare Drug Plan?

Initial Enrollment Period (IEP)

You can first enroll in a Medicare drug plan when you’re newly eligible for Medicare — starting three months before the month you turn 65, and ending three months after.

Annual Election Period (AEP):

This is when most people make changes to their Medicare coverage. You can enroll in, switch, or drop a Part D plan during this time. Changes take effect on January 1.

Special Enrollment Periods (SEPs)

If you lose creditable drug coverage (like from an employer), move out of your plan’s service area, or face other qualifying life events, you may qualify to enroll or make changes outside the standard windows.
